I, the undersigned, hereby grant Beats By Kimmie (hereinafter referred to as the “Makeup Artist”), the irrevocable and unrestricted right to use, reproduce, and publish photographs and/or video recordings of me taken before, during, and after makeup services.

I have answered the questions on this form truthfully and understand a Bridal Wedding contract with signature is required before any services will be rendered. I have familiarized myself with the cancellation policy and understand that ALL Depostis or UPFRONT MONIES paid are NON -REFUNDABLE.
